Excessive acid concentration or frequency of use can excessively wear the salt cell - you didn't mention how long you soaked the cell in the full strength acid . . .
The coincidence of low salt and cell maintenance lights is a classic indication of a worn out cell. Based on the cell condition before that last cleaning, its age, and/or the time in the acid, it might have been the straw that broke the camel's back.
